The companion 2 driver will be returned to syncardia for evaluation.The results will be provided in a follow-up mdr.(b)(4) initial.
 
Event Description
The companion 2 driver was not supporting a patient.The customer, a syncardia authorized distributor, reported that the companion 2 driver did not recognize the external batteries when the driver was powered on.
 
Manufacturer Narrative
The patient data file was reviewed and revealed multiple battery alarms and notifications that could be either from lack of recognition, as reported by the customer, or from the external batteries being removed as part of the system checkout.The customer did not return the external batteries used at the time of the reported issue.During investigation testing, the companion 2 driver passed all functional testing, including additional testing using several external batteries that have previously met performance requirements.The root cause of the companion 2 driver not recognizing the external batteries could not be determined.The driver performed as intended with no evidence of a device malfunction.This issue will be monitored and trended as part of the customer complaint process.Syncardia has completed its investigation and is closing this file.Ce 5752 (b)(4) follow-up report 1.
