The suspect device has been returned to olympus for evaluation, the service center observed did not confirm the reported event as the lamp illuminated properly at maximum output.It was noted a non-olympus lamp was installed in the unit.Additionally, the lamp was not seated properly into the heat sink and the lamp bolts/washers were mis-aligned.The investigation is ongoing, and a definitive root cause of the reported event cannot be determined at this time.If additional information becomes available, this report will be supplemented accordingly.
 
Event Description
The customer reported to olympus that during preparation for a procedure the evis exera iii xenon light source illumination is low when turned in the max position and is not bright.There was no patient death, harm, or injuries reported with this event.
 
Manufacturer Narrative
This supplemental report is to inform that upon further review, this is not a reportable malfunction.Per the legal manufacturer, there is no potential for this issue to cause or contribute to death or serious injury if the malfunction were to recur.
