Event Description
During a regular maintenance inspection, the customer reported that the device when switched on starts flashing and alarming and then switches off.There is no patient involvement associated on this reported event.No user injury reported.

The subject device has not yet been received for evaluation.The cause of the issue cannot be determined at this time.If additional information becomes available this report will be supplemented accordingly.Investigation is ongoing.This report will be supplemented accordingly following investigation.

This report is being supplemented to provide additional information based on the approved final investigation and device evaluation.The device was evaluated and the customer's complaint was not confirmed.However, when the unit was switched on the front panel led's switched off and the alarm sounded.This was found to be due to a faulty circuit board.The olympus repair center also found the chassis was rusted in multiple places but this defect is not a potential adverse event.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Based on the results of the investigation, the definitive root cause of the faulty circuit board could not be determined.Olympus will continue to monitor field performance for this device.
