While troubleshooting the customer stated they recently replace the xenon lamp on the unit upon powering on the unit emergency lamp start blinking then stops.They then compared the start up process against a secondary device.The service center tech was then informed once xenon lamp was reseated the issue was resolved.The investigation is ongoing, and a definitive root cause of the reported event cannot be determined at this time.If additional information becomes available, this report will be supplemented accordingly.
 
Event Description
The customer called olympus to request information on the emergency lamp function on their evis exera iii xenon light source unit.The were no reports of death, harm, or injuries related to this event.
 
Manufacturer Narrative
This report is being supplemented to provide additional information based on the legal manufacturer's investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 6 years since the subject device was manufactured.Based on the results of the legal manufacturer's investigation, the root cause of the emergency lamp display blinking could not be identified.However, it is likely the cause is related to the installation of the xenon lamp.Olympus will continue to monitor field performance for this device.
