Event Description
According to the reporter, during hysteroscopic myomectomy, they noticed what looked like glitter pieces in the cavity, seemed to be metal coating from the shaver.Surgeon decided to continue but change out for new device.Used another dense tissue shaver mini.Issue did not proceed with new device and from what did they see was shaver sucked in fragments.There was no patient injury.
