It was reported that there was an issue with pl574t- challenger ti-p sm-ligat.Clips 12 cartr.According to the complaint description, intraoperatively the ligation clips jammed.An additional medical intervention was necessary.Additional information was not provided nor available.Additional information has been requested but not yet received as of this report.Additional patient information is not available.The adverse event is filed under aag reference (b)(4).

Investigation results: three products arrived in a sealed condition.One product arrived in a used condition with a clip jam.We made a visual inspection of the products, here we detected a clip jam and deformed latches of the slider sheet.Furthermore, we have tested the complained sealed cartridges and could not detected any deviations.Batch history review: the device quality and manufacturing history records (dhr) have been checked for all leading device(s) lot numbers and the products found to be according to our specification valid at the time of production.There are no similar complaints against the same lot number(s) with this error pattern.The review of risk assessment revealed that the overall risk level (severity 3(5) x probability of occurrence 1(5)) according to din en iso 14971 is still acceptable.Explanation and rationale: if the cartridge is engaged not completely or was damaged during inserting, there is an impairment of product functionality.This could lead to deformed latches of the slider sheet, a clip jam and can cause the cartridge to detach.Too fast application can be another cause.Based upon historically grown product experience and due to different simulation regarding a too fast application or a cartridge which is engaged not completely, this leads to the described errors.Possibly a damaged clip applicator due to a deformed push rod or something similar could be another cause for wrong positioned clips or clip jam.Conclusion and measures / preventive measures: based upon the investigation results a clear root cause conclusion cannot be drawn.There is no indication for a material-, manufacturing- or design-related failure.Based upon the investigations results a capa is not necessary.
