Event Description
|
An attempt was made to use one everest inflation device during a procedure to treat a non-tortuous and non-calcified lesion located in the pulmonary artery.The device was inspected with no issues noted.The device was prepped per ifu with no issues noted.The indeflator was connected to three balloon devices which were in the patient's body.It was reported that balloon rupture occurred with each device.Pressure was applied from the range of nominal burst pressure (nbp) to rated burst pressure (rbp), however balloon ruptures occurred.It was stated that the actual pressure applied may have been higher than the pressure value that was displayed on the pressure gauge.The patient is alive with no injury.
