Event Description
Information was received from a patient who was implanted with an implantable neurostimulator (ins) for unknown indications for use.It was reported that something is wrong with her old device. the one that implanted (b)(6) 2019 is not in the correct place as per the patient and that is the reason why they replace it with a new implant.

Event Description
Additional information was received from the patient.They reported that when they indicated that the device was not working, they indicated that it was due to therapy or symptom control.The cause was not due to normal battery depletion.The cause of the device not working was determined.The cause was determined to be due to hip replacement, pulling or stretching, dislodged probe.They had a procedure done to attach the probe, but they also upgraded their device at the same time as well.The indicated the issue has been resolved.
