A dhr review was conducted which indicated all inspections were completed and no issues were noted during manufacture.Visual inspection was performed on 4 pictures and 8 samples received in new conditions inside their closed original packaging.No particles were found in the pictures received.2 of the returned samples no contamination was observed.6 remaining samples the threshold of contamination was not observed.No root cause could be determined since the complaint was not confirmed because samples meet the specification.No corrective actions were taken since the complaint was not confirmed.
 
Event Description
It was reported foreign material speck between bladder and wall of cassette.No patient involvement was reported.
 
Manufacturer Narrative
While performing a review of submitted mdr report it was discovered that this file was inadvertently assessed as reportable.No patient death, serious injury, and no evidence of a reportable product malfunction.The hazardous situation for the given complaint device would not likely cause or contribute to a death or serious injury if the malfunction were to recur.File (b)(4) is no longer considered reportable, please disregard any mdr reports associated with it.
