The device is returned and an evaluation completed for it.The user¿s complaint was confirmed.Upon inspection and testing, it was observed that the front panel button does not respond.This is attributed to the main circuit board failure.There is also lighting failure due to the failure of the front panel light-emitting diode (led).Evaluation is ongoing.Supplemental report(s) will be submitted when any relevant new information is available.

Addendum may 10, 2022: the event occurred during preparation for use inspection prior to a procedure.The intended procedure was completed with no delay.

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.A review of the device history record found no deviations that could have caused or contributed to the reported issue.It has been over 8 years since the subject device was manufactured.Based on the results of the investigation, the button on the front panel became unresponsive due to the failure of the main board.As a result, it is likely the failure of the led lighting occurred due to the front panel failure.It was not possible to identify the cause of the main board failure.Olympus will continue to monitor field performance for this device.
