Event Description
The device involved a transpac iv monitoring kit which was reported to have the tubing separated from the transducer which resulted in leakage of normal saline.The incident was noted during preparation.There was no patient involvement or harm reported as a consequence of this event.

The device has been received for evaluation, however, investigation is not yet complete.

One used transpac¿ iv monitoring kit, disposable transducer, 3ml squeeze flush device, macrodrip was received for evaluation.An image was provided by the customer showing the involved product.The reported complaint of tubing separation was confirmed on the returned set.During visual inspection, the pvc tubing was received separated from the winged male luer.When the tubing pocket was microscopically examined, sufficient solvent coverage appeared to be present on the tubing.The probable cause for the disconnection between pvc tubing and the male luer had occurred due to insufficient solvent coverage on the tubing during assembly process.The lot history was reviewed and no nonconformities were identified that may have contributed to the reported complaint.
