It was reported that during thoracoscopy esophagectomy, partial gastrectomy by videolaparoscopy and cervical esophagogastroplasty, the enseal tweezers presented a defect and ceased to function in the middle of the procedures (it did not seal and warned that it had not completed the coagulation process), requiring the use of an ultrasonic harmonic tweezers to complete the procedures.

(b)(4).Batch #: u95615.Additional information was requested and the following was obtained: has the patient suffered any adverse consequences? no investigation summary: the product was returned to ethicon endo surgery for evaluation.Visual inspection and functional testing were conducted on the returned device.Visual analysis of the returned sample determined that the nslx137c device was returned with the jaws misaligned.Upon visual inspection to the jaws it was observed that one side of the hinge pin was missing.The hinge pin missing was not returned with the device.The device was tested on the generator, this resulted in the ¿reposition jaws and reactivate¿ alert screen, this is advising the generator cannot deliver energy.Then the "replace instrument" screen would be displayed after receiving the "reposition jaws and reactivate" message three time.The hinge pin missing could have contributed to the reported ¿tissue effect issues¿ , this due to the reduced compression capacity of the jaws.Based on the information currently available, a product issue was identified during the investigation of the sample received.This product issue will be addressed through ethicon endo surgery¿s quality system.A manufacturing record evaluation was performed for the finished device batch number, and no non-conformances related to the reported complaint condition were identified.If information is obtained that was not available for the initial report, a follow-up report will be filed as appropriate.
