The results of the investigation are inconclusive since the reported device was not returned for analysis.Based on the information received, the cause of the reported event could not be conclusively determined.The material inspection report for this guide wire lot number has been reviewed.No issues or discrepancies were noted during this review that would have contributed to the reported event.The device met material, assembly, and quality control requirements.Csi id: (b)(4).
 
Event Description
A viperwire advance peripheral guide wire was used with a 1.25mm micro crown orbital atherectomy device (oad) for treatment of a lesion in the radial artery.Access was obtained in the groin, and a 120cm guide catheter was used.The viperwire had been advanced into the vasculature of the hand.The oad was inadvertently advanced over the spring tip of the wire during treatment.When the oad was removed, the tip of the wire remained in the radial artery.A stent was deployed over the fragment, and the hand had sufficient flow thereafter.The patient was well at the conclusion of the procedure.
