The device was not returned for analysis.A review of the lot history record identified no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no similar incidents from this lot.The investigation was unable to determine a conclusive cause for the reported deflation failure.The reported difficulty to remove appears to be related to operational context of the procedure as it is likely the removal of the device without full deflation of the balloon caused the reported difficulty to remove.There is no indication of a product quality issue with respect to manufacture, design or labeling.
 
Event Description
It was reported that the procedure was to treat a 90% stenosed lesion in the left main coronary artery (lmca).The 4.0x18mm xience prime stent delivery system (sds) was advanced to the target lesion and the balloon of the sds was inflated one time to nominal pressure, successfully deploying the stent; however, the balloon failed to deflate.Negative pressure was held for 8-10 seconds and the contrast mix was 1:1.The sds was removed with the whole assembly with difficulty as it was not deflated.There was no adverse patient effect and no reported clinically significant delay.No additional information has been provided.
