A lead extraction procedure commenced to remove a right atrial (ra), a right ventricular (rv) and a left ventricular (lv) lead due to cied system/pocket infection.Spectranetics lead locking devices (llds) were inserted into each lead to provide traction.All leads were successfully extracted with use of a spectranetics glidelight laser sheath, with the lv lead being the last lead removed.During the ten minute cool down phase after the procedure had completed, an effusion started to form.Rescue efforts began, including sternotomy.A coronary sinus perforation was discovered and successfully repaired.The patient survived the procedure.It was suspected that traction applied to the lv lead perforated the heart muscle.This report captures the lld providing traction to the lv lead when a coronary sinus perforation occurred, requiring intervention.There was no alleged malfunction of any spectranetics devices in use during the procedure.
