Event Description
Injuries - mouth [mouth injury].Brush heads no longer stay securely attached to the hand piece.Constantly gets pulled off the hand piece - oral-b [device breakage].Worn-down hand piece - oral-b [device physical property issue].Coming loose - oral-b [device connection issue].Case narrative: male consumer via e-mail stated that after approximately 3 years of usage, the oral-b toothbrush heads (even brand-new ones) no longer stayed securely attached to the oral-b hand piece.While brushing his teeth, the oral-b toothbrush head constantly got pulled off of the oral-b hand piece, which quickly led to injuries.It was a worn-down hand piece.No serious injury was reported.On 23-may-2022 follow up via e-mail: the suspect product lot was 1 cd920 22002.The consumer reported that the oral-b toothbrush heads kept coming loose.No serious injury was reported.

26-sep-2022 product investigation results: product return was received and investigated.Product investigation results showed that the complaint is caused by a breakage of the tip of the driving shaft due to improper consumer handling, storing and cleaning of the product.
