Event Description
According to the reporter, during laparoscopic hysterectomy, the needle was identified using x-ray but the surgeon could not locate the needle based on x-ray information.The surgeon felt like more damage could be caused by trying to find and remove the needle and decided to leave the needle in half.The surgeon thought the needle was lodged in vaginal cuff tissue.To resolve the issue, a new reload was used and completed cuff closure.Surgical time was extended for more than 30 minutes.Waiting and performing x-ray.Checking for location of needle.
