A facility reported that the mayfield skull clamp (a1059) slipped while in the locked position and lacerated the patient's face.The cut was mild and patient was doing okay.The slip caused less than a 10 minute delay in surgery.The facility subsequently advised that that after inspection, they determined that the skull clamp did not fail.Instead, the pins were not properly secured into the skull clamp which is what caused the slip; therefore, it was user error that caused the incident.

The mayfield modified skull clamp (a1059) was not returned for evaluation; therefore, an evaluation of the device could not be performed.Lot number information has been provided; therefore, device history record (dhr) was reviewed, and no anomalies were found.Customer called integra shortly after reporting the complaint, and stated that "after inspection, the skull clamp did not fail after all.Instead, the pins were not properly secured into the skull clamp which is what caused the slip; therefore, it was user error that caused the incident." complaint not confirmed, operator error.
