Event Description

Physician was attempting to use an abre self-expanding stent along with a 9fr sheath and non-medtronic guidewire during procedure to treat a lesion in the left mid external iliac vein (eiv) going into the common femoral vein (cfv).No embolic protection was used.There was no damage noted to packaging.There was no issue noted when removing device from hoop/tray.The device was prepped per ifu with no issues identified. it was reported that during balloon inflation, inflation difficulties occurred.A 14mmx60mm abre stent was deployed successfully in the external iliac/common femoral vein.Stent looked great after placement.Post dilation occurred and the balloon detached from the delivery system requiring forceps to remove the detached balloon.During this process the abre stent was mangled and needed to be removed.Interventionalists felt confident the stent could be retrieved from the body.Stent was successfully removed through the sheath and pulled out of the body.There was no patient injury.
