According to the reporter, the catheter's red or blue end had developed a split/crack (back ends).There was no available repair kit and the catheter remained in use.There was no reported patient outcome.

Event Description
|
According to the reporter, after 9 months, 1 week and 5 days, the catheter's red (arterial) and blue (venous) luer adapters (back end) had developed a split/crack and it was documented on july 22, 2022.There was no leak.Tego was not utilized.Clinell 2% chlorhexidine in 70% alcohol individually wrapped wipes, unisurge pack line on (3ml and 5ml syringes), gambro artiset blood tubing system for hemodialysis with gambro artis, unisurge pack line off (3ml and 5ml syringes) with red uhs obturator cap, bd plastipak 20ml luer-lok syringe, bd vacutainer multiple sample luer adapter, bd vacutainer one-use holder, bd vacutainer blood collection tubes were the products utilized with the device.Clinell 2% chlorhexidine in 70% alcohol individually wrapped wipe was the cleaning agent used onthe device and it was typically utilized to clean the adapters.The adapters were tighten by hand.Nothing unusual documented at time of insertion.Flushing at time of insertion was not documented.The catheter had heparin lock withdrawn and lumen flushed prior to every use.No excessive force used on the catheter.There was no available repair kit as it was out of stock.The manufacturer was contacted for advice on next steps, and datix (safety report) was completed.Quality and safety team of the hospital investigated, and dated photographs taken of both catheter lumens.New monitoring assessment tool was made to carryout daily assessment of lumens and catheter.Renal team researched different brand of hemodialysis catheter to replace the catheter.The dialysis catheter would need to be removed and replaced if appropriate due to the risk of infection and air embolus.The dialysis catheter was removed.There was no blood loss.There was no patient injury.
