Event Description
|
It was reported that during the cardiac resynchronization therapy defibrillator (crt-d) implant procedure the patient had experienced atrial fibrillation (af) but that it stopped safely during the operation and the procedure was completed successfully.It was noted that the patient had a history of heart failure and immediately after the procedure it seemed the patient¿s condition had improved, however several days later after af recurred the patient¿s condition worsened again, and the patient was cardioverted five times which stopped the af.After that, the patient¿s heart rate decreased to forty beats per minute which went unnoticed by the hospital and the patient passed away at six days post implant.

Additional information received indicated that prior to the procedure the patient's condition was not good and they were at the level of requiring a heart transplant.It was also noted that initial af and cardioversion happened prior to the procedure with none actually occurring during the procedure.Finally, it was noted that following the cardioversions that occurred after the procedure there were no pacing abnormalities seen from the device.
