Event Description
|
It was reported that prior to a cryo ablation procedure, the dilator could not be placed within the sheath.The dilator was removed from the hub and all components were wiped down with saline before attempting to insert the dilator again; the issue persisted.The sheath was replaced and it was observed that there were shards of plastic that had broken off of the dilator and that there was a milky white substance.The dilator was replaced which resolved the issue.There was no patient involvement.
