Event Description
|
This is being filed for compressive stress that caused damage to the soft tip.It was reported that on (b)(6) 2022, a patient presented with grade 4 mixed mitral regurgitation (mr) and a bilateral femoral bypass.The sgc was damaged during an attempt to access the puncture site.The access puncture was predilated with 16 f and 18 f dilators.When attempting to insert the sgc, it was impossible to advance.The sgc tip was visibly bending on the fluoroscopy as it was inserted into the femoral vein.The sgc was set aside to further dilate with a 20 f dilator.While inspecting the device, soft tip damage was noted.The surface was wrinkled and the curve was not smooth.A replacement completed the procedure successfully, and the mr was reduced to 2.There was no adverse patient effect or clinically significant delay.No additional information was provided regarding this issue.

The device was not returned for analysis.A review of the lot history record revealed no manufacturing nonconformities issued to the reported lot that would have contributed to this event.Additionally, a review of the complaint history identified no similar complaints reported from this lot.Based on available information, the reported difficulty inserting the steerable guide catheter into anatomy appears to be due to procedural conditions.The reported sgc tip deformation appears to be a cascading event of the difficulty inserting the sgc into anatomy.There is no indication of product issue with respect to manufacture, design or labeling.
