Device evaluation details: the product was returned to biosense webster inc (bwi) for evaluation and the evaluation has been completed.Bwi conducted a visual inspection and carto 3 system test of the returned device.Visual analysis of the returned catheter revealed a hole on the pebax of the device and a reddish material inside.A magnetic sensor functionality test was performed, and the device was recognized on the system; however, error 105 was displayed on the screen due to an open circuit on the tip area.As part of bwi¿s quality process, all devices are manufactured, inspected, and released to approved specifications.A manufacturing record evaluation was performed for the finished device batch number, and no internal actions were identified.The instructions for use contain the following warning stated in the carto 3 system manual: the magnetic sensor of the catheter is disconnected.If the problem persists, replace the catheter cable or the catheter.The date of event was not provided.As such, date of event has been populated with (b)(6) 2022.If additional information is received regarding this event, a supplemental 3500a report will be submitted to the fda.Manufacturer's ref.# (b)(4).
 
Event Description
A thermocool® smart touch® sf bi-directional navigation catheter was returned to biosense webster inc (bwi)'s product analysis lab (pal) labeled with a complaint number for which product details, such as the product catalog number and the lot number do not match.Multiple attempts have been made to obtain clarification for the wrong product received, however, no further information has been made available.A new complaint was created to investigate and report the malfunction found during product evaluation although no specific event details are available.On 19-jul-2022, the returned catheter was inspected, and it was found with a hole in the pebax exposing internal parts and a reddish-brown material inside the pebax.These findings were reviewed and determined the issue of a hole in the pebax is an mdr reportable malfunction since the integrity of the device has been compromised.
