Investigation summary: a device history record review was completed for provided material number 306572 and lot number 1351585.The review did not reveal any detected abnormalities during the production process that could have contributed to the reported defect and all quality tests were found to be within specification.As a sample was unavailable for return, a thorough sample investigation could not be completed.Based on the limited investigation results, an exact cause could not be determined for the reported incident.However, per the provided feedback, it is possible that the incident resulted from customer misuse.The bd posiflush xs syringe is intended for flushing only.Further action has not been determined necessary at this time.Our quality team will continue to closely monitor the manufacturing process for signs of this potential defect and any emerging trends.
 
Event Description
It was reported that the bd posiflush¿ xs pre-filled flush syringe nacl 0.9% experienced leakage.The following information was provided by the initial reporter: "the customer states that after spiking the normal saline with a magnesium bag, there was a small leak that was difficult to identity.The nurse wasn¿t sure what caused the leak as it was nothing visible, it could be the bag as well as the giving set, according to him.".
