Event Description
|
The customer reported to olympus, at the end of a therapeutic (laparoscopic) procedure, when the stop button of the high flow insufflation unit was pressed, an error sound was generated, and the front panel blacked out.Subsequently, the device was turned off and on to resolve the issue.There was no harm or user injury reported due to the event.Additionally, the customer stated the issue has occurred previously.The previous incident is recorded under the medwatch report with patient identifier: (b)(6).

This report is being supplemented to provide additional information based on the legal manufacturer's final investigation.It has been over 9 years since the subject device was manufactured.Based on the results of the legal manufacturer's investigation and based on a similar complaint with a returned product, it was determined that the display on the front panel was blacked out due to the failure of the main board.It was confirmed that when the stop button was pressed in an attempt to stop gas feeding of uhi-4, an error sound was generated, and the front panel blacked out.A review of the device history record found no deviations that could have caused or contributed to the reported issue.Olympus will continue to monitor field performance for this device.
