When observing the product it was confirmed that the base of the male connector of the flat filter was broken and the fracture surface was whitened.It was determined that excessive force was applied to the filter, which was the cause of whitening, when the plastic material was broken by an external force and was observed.No lot number was provided; therefore, device history record review could not be completed.D4 and g5 are unknown.No product information has been provided to date.This remediation mdr was generated under protocol (b)(4), as a result of warning letter (b)(4).
 
Event Description
It was reported that the customer indwelled the product in the patient during the operation.They then transported the patient to the hospital ward and noticed the device connector was broken.No patient injury was reported.
