A device history record (dhr) review was performed subsequent to the manufacturing of the device and prior to its release.No problems or issues were identified during this dhr review.One device received by the investigation site.Visual inspections showed the enclosure was dirty, missing rubber feet, pole clamp was dirty, line cord is old and worn, reservoir tank is cloudy, cracked enclosure underneath drain fitting, the liquid crystal display (lcd) is damaged with liquid crystal leakage, contains an old style printed circuit board (pcb), and drain fitting.The primary source of the reported issue was a faulty pcb board.No action taken due to the age and condition of the device.It is deemed beyond economical repair and will be scrapped.Udi information is unknown.This remediation mdr was generated under protocol (b)(4), as a result of warning letter cms# 617147.
