It was indicated that the device will not be returned for evaluation.If there is any further relevant information obtained, a supplemental medwatch will be filed.
 
Event Description
It was reported that a blazer ablation catheter was used in a procedure.Post procedure it was noted that the patient developed cardiac tamponade with confirmed pericardial effusion via intracardiac echo (ice).A interventional cardiologist summoned and completed pericardiocentesis successfully.Pulseless electrical activity (pea) was observed and cardiopulmonary resuscitation (cpr) was performed.The patient left intubated on pressor and responding.A while later the patient was extubated.Performed a standard ep study prior to transeptal puncture, which was done without any issue and completed multiple radio frequency (rf) ablation lesions after which accessory pathway was terminated.
