The pacing catheter will not be returned for evaluation, as it was discarded at the facility.Without the return of the unit, it is not possible to determine if some damage or defect existed on the unit that could have contributed to the event.It is not known if some procedural factors may have contributed to the event.A device history record review could not be completed because the lot number is unknown.Complaint histories for all reported events are reviewed against trending control limits on a monthly basis, and any excursions above the control limits are assessed and documented as part of this monthly review.Device discarded at facility.

It was reported that during use, a physician was unable to get the pacing catheter (model d97120f5) to work.The pacer wire was inserted and turned on, but the device would not capture.It was removed and another device was used.The exact date of the event and the lot number are unknown.There was no patient injury.The product is not available for return as it was said to have been discarded.
