Event Description
It was reported that during a primary implant today, the setscrew from two sensight extensions dislodged during tunneling and remained in the extension track (approximately lower neck, subcutaneous).Spare setscrews were utilized to lock the lead into the extension.The surgeon opted to leave the dislodged setscrews in the body.If the patient cannot have an mri, it would require a further operation to attempt to explant the setscrews.Refer to manufacturer report 2182207-2022-01836 for related device.

Event Description
Additional information was received: it was reported that the scrub nurse was used to using boston scientific extensions where they back the screw our before insertion of the dbs lead.Unfortunately the nurse also backed the screws out for our extensions, enough so they just stayed in place until they were pulled through the neck where they became dislodged.
