Event Description
It was reported that the twist clamp (white sleeve) of a minicap transfer set was damaged; further described as ¿no flute on white sleeve¿ resulting in ¿a gap after twisting¿ (gap between sleeve and mainbody).This was observed during before use of the device for peritoneal dialysis therapy.There was no patient involvement.No additional information is available.
 
Manufacturer Narrative
H10: the device was received for evaluation.A visual inspection with the naked eye observed the lock notch on both sides of the twist clamp was not present.The reported condition was verified.The direct cause was determined to be related to a manufacturing issue.This condition would not result in a breach of the sterile pathway; therefore, would not lead to a serious injury.A batch review was conducted and there were no deviations found related to this reported condition during the manufacture of this lot.Should additional relevant information become available, a supplemental report will be submitted.
