Event Description
It was reported that after an iol was placed at into the left eye, it rotated approximately six weeks post-operatively.Lens was repositioned to original two months post-op and with toric sitting in the correct position.Reportedly vision is great.

The device remains implanted and is not available for evaluation.The investigation is ongoing and a follow-up report will be submitted upon completion of investigation.

A review of the device history record (dhr) did not find any non-conformities or anomalies related to this event.The lot history, trend analysis, risk analysis and/or directions for use review were considered acceptable, with the product performing within anticipated rates.Based on the information provided, a root cause was unable to be determined.
