Event Description
It was reported that the during the right procedure, the target was checked with lp, and after inserting and placing the sensitization lead in the intracranial, an attempt was made to remove the stylet, but an event occurred in which a load was applied with the handle and only a little bit could be removed.The lead fixation site of the mtd was confirmed, but there was no improvement.Under the supervision of the physician who came to instruct that the support clip was anchoring and the stylet was removed.(physician said that the load did not change much.) at that time, lead misalignment occurred.An attempt was made to reinsert the stylet that was removed with the lead, however, the shape was bent and it could not be inserted.A new lead was taken out.The issue was resolved.
