It was reported that a vitality lenke probe fractured during a case.All pieces were successfully recovered from the patient anatomy.Another probe was used to complete the case without patient impacts.

Additional information in h6: component, investigation type, findings, and conclusions.Device evaluation: the device was not returned, but a photo was provided.The photo shows that the tip of the probe broke off in the area of the flat-to-round transition.Potential cause: root cause was unable to be determined.This event could possibly be attributed to wear through use over time or multiple sterilization cycles.It could also be attributed to the hardness of the patient's bone.Dhr review: dhr review unable to be performed as lot information is not known.Device use: this device is used for treatment.A follow-up report will be submitted if new information is received that changes the information provided in this report.
