Event Description
A physician was using a turbohawk plus atherectomy device for the treatment of a calcified plaque lesion in the superficial femoral artery with moderate tortuosity.It was an antegrade stick in a calcified artery.There was moderate tortuosity and calcification.The device was prepped as per the ifu with no issues identified.It was reported the turbohawk plus was used successfully, but when the physician was removing the device, resistance was felt when the device reached the sheath.Upon removal it was reported that the nosecone detached.A second access was gained, and the nosecone was jailed against the artery wall with a stent.No patient injury reported for this event.
