Event Description
Medtronic received information that prior to use of a fusion oxygenator during setting of the circuit, it was reported that the perfusionist noted a little damage to the out flow in the oxygenator.  the device was replaced.There was no patient involvement, so no adverse effect occurred.Medtronic received additional information that there was a very small crack on the edge of the 3/8" connection.There was no damage to the packaging (outer box, inner packaging or sterile barrier).The other devices in the same box/shipping container were not damaged.
