It was reported that the healthcare provider (hcp) interrogated the patient's implantable neurostimulator (ins) today and found a possible fault on the left vim electrodes 0 and 1.Patient is experiencing symptom return including tremor on the right side of his body.No environmental/external/patient factors that may have led or contributed to the issue were identified.Hcp repeated the impedance check several times with the same results.Patient was sent for skull and chest x-rays to visualize the implanted components further.Impedance check was repeated.Hcp attempted reprogramming to avoid electrodes 0 and 1 but patient symptoms did not resolve.

Additional information received from the manufacturer¿s representative (rep) reported surgical intervention took place on january 9th where the neurostimulator pocket and subgaleal pockets were accessed and troubleshooting was performed.All impedances were checked multiples times, and all were now within normal range.
