Event Description
It was reported that during a cryo ablation procedure, during the second freeze of the right superior pulmonary vein (rspv) the patient was hypotensive.After another freeze the patients blood pressure dropped further.Cardiopulmonary resuscitation (cpr) was commenced and the medical response team (mert) contacted.Pulseless electrical activity (pea) was confirmed.Intensive care unit (icu) support arrived, and shocks were delivered.The patient was placed on extracorporeal membrane oxygenation (ecmo).Imaging was carried out and confirmed the patient had a retro-peritoneal bleed and intervention took place to stop the bleeding. the case was aborted under general anesthetic. the patient was brought to intensive care unit (icu) and hospitalization was extended.The patient then had several cardiac arrests over the next couple of days, requiring the implantation of a temporary pacemaker.The patient went into pulseless electrical activity (pea) the following morning and subsequently expired.
