Event Description
|
According to the reporter, during a laparoscopic cholecystectomy, while at blood vessel closure, the clipping was done in first firing, but the clip could not be loaded correctly after clipping on top of the clip for the second firing.A new device was used to complete the case and resolve the issue.There was no patient injury.
