Event Description
A physician was using a hawkone atherectomy device for the treatment of a soft tissue lesion in the mid superficial femoral artery.The artery diameter was 6mm and there was mild tortuosity.A 7fr non-mdt sheath was used and a 6mm embolic protection was used.The vessel was not pre-dilated.The ifu was not followed - 'in stent' section not followed.It was reported resistance was felt during withdrawal and once removed it was noted that there was damage to the nosecone and cutter was against the mesh.The catheter did not experience difficulty locking into the cutter driver.The cutter driver stopped functioning while device in use in patient.The thumbswitch partly turned off.The cutter was inside housing/outside housing but pushing into the mesh wall just below the cutter window during device removal from the patient.The kink was observed on the nosecone when the device was removed.A dcb was used to complete the procedure as they were finished using the hawkone.The device was safely removed from the patient.No patient injury reported.
