Event Description
|
According to the reporter, on a laparoscopic cholecystectomy, when connecting the gallbladder vessels, the clips were malformed and fell into the cavity.Vascular closure could not be performed.The clips were retrieved with the help of a holder.A second device from the same lot was used, but also had the same issue.The surgical time was extended for 30 minutes due to the issue.A third clip applier from another lot had the issue.Another device was used to complete the case.There was no patient injury.
