Event Description

According to the reporter, during the procedure, the physician tried to insert the titanium adapters, using hand technique and no devices was used.After the peritoneal dialysis (pd) catheter was completely inserted, the two-piece titanium adapters were not able to be properly tightened together because it did not entirely engage and there was space (a gap) between the two pieces.Although they removed both pieces off and tried again without the catheter, there was still a gap between the two pieces and did not engage the catheter was not repaired and had no leak.Tego was not used, and there was no problem with the luer adapter.There was no cleaning agent used on the device and no other products being utilized as well.There was no visible defects/damages found on the titanium adapter and the procedure was completed and successful with no issue using another titanium adapter.There was no blood loss and no remedial action was done.The patient did not have any medical intervention/treatment due to the event.There were no patient symptoms or complications associated with this event.There was no reported patient injury.
