Event Description
A physician was attempting to implant an eveflex entrust for the treatment of a plaque lesion in the mid region of the superficial femoral artery.There was moderate calcification and moderate tortuosity.The patient had an extremely steep bifurcation and there were issues with placing the sheath.A 6fr non mdt sheath was used.The vessel was pre-dilated with an evercross balloon.An entrust stent was attempted to be implanted after, the device was prepped as per the ifu with no issue identified.Once the stent was in position the lock-pin was removed and the stent partially deployed. the stent fractured and detached from the stent delivery system (sds).The sds then became stuck on the wire.The stent would not continue deploying.The sds and the wire were removed from the patient.The fractured stent remained in patient.Physician used a 6x120 protégé everflex to continue the stent and then used a 6x150 inpact admiral dcb.No patient injury reported for this event.
