Date of event and udi section, are unknown, no information has been provided to date.A supplemental mdr will be filed as necessary in accordance with 21 cfr 803.56 when additional reportable information becomes available.A product sample was received and is awaiting evaluation, investigation including root cause analysis is in progress.
 
Event Description
It was reported that the warmer doesn't alarm and the led light doesn't come on.There has been no report of patient involvement or no observable clinical symptoms or a change in symptoms identified in the patient.
 
Manufacturer Narrative
Device evaluated by manufacturer and event problem and evaluation codes: updated.One warmer device was received for investigation.Damage to the enclosure, tank cover, and front cover was observed.The line cord was loose.The printed circuit board and power switch were determined to be outdated.The reported issue was confirmed during functional testing when the warmer alarms did not produce an audible alarm, and the lcd screen was darkened.The issue was traced to the printed circuit board and its connected speaker, which were determined to be defective.The root cause was attributed to wear from usage.As no manufacturing related root cause could be identified, no review of device history records was conducted.The device was determined to be beyond economical repair and will be decommissioned.
