Event Description
It was reported that during exploratory laparoscopy, the device worked while laparoscopic, converted to open and the device stopped working it was unplugged kept sterile plugged back in and worked.After an unknown amount of time the device stopped working again.Case was completed with a second device.No patient harm was reported.

(b)(4).Batch #: unknown.Attempts are being made to obtain the following information.To date, no response has been provided.If further details are received at a later date, a supplemental medwatch will be sent: can clarification be provided of what is meant by ¿stopped working¿ what was the reason for the conversion to open? is there an alleged deficiency with the first device that may have caused or contributed to the decision to convert to open? were there any generator alert screens? if yes, what were they? was there only 1 generator involved in the case? was the device plugged into 1 or more generators during the procedure? an analysis of the product could not be performed since a physical sample was not received for evaluation.An evaluation of the manufacturing record could not be performed as the required product identification number was not provided to complete the evaluation.As part of our company quality system process, all devices are manufactured, inspected, and distributed to approved specifications.This report is being submitted pursuant to the provisions of 21 cfr, part 803.This report may be based on information which has not been investigated or verified prior to the required reporting date.This report does not reflect a conclusion by ethicon, or its employees that the report constitutes an admission that the product, ethicon, or its employees caused or contributed to the potential event described in this report.If information is obtained that was not available for the initial report, a follow-up report will be filed as appropriate.

(b)(4).Date sent: 3/1/2023.Additional information obtained: unfortunately, i have no additional information.When they mentioned it stopped working, they meant the device did not activate energy.This is how it was reported to me.I don¿t believe the decision to open had anything to do with the device not working.
