Event Description
It was reported that on (b)(6) 2023, a 19mm epic plus supra valve was selected for an implant.During the procedure, while implanting the valve, the sutures came out from one side of the holder.The surgeon eventually managed to implant the same valve with no consequence to the patient.The patient remained hemodynamically stable throughout the procedure.The patient was reported to be in stable condition.

An event of sutures came out from one side of the holder was reported.Information from field indicated that the event occurred while implanting the valve.One video was received from the field, appearing to be taken during implantation when the reported incident was observed.A returned device inspection, to rule out any device-related causes, could not be performed as the device was not returned for analysis.The device history record was reviewed to ensure that each manufacturing and inspection operation was performed and the product met all specifications.Based on the information received, the root cause of the reported incident could not be conclusively determined but is consistent with the valve coming into contact with a sharp object during implantation.There is no indication of a product quality issue with regards to manufacture, design, or labeling.
