Event Description
|
An attempt was made to use one onyx frontier coronary drug eluting stent to treat a non-calcified, non-fibrotic lesion.There was no damage noted to packaging.There were no issues noted when removing the device from the hoop/tray.The stent did not have anything damaged once removed from the package.The device was inspected with no issues noted.Negative prep was performed with no issues noted.The lesion was pre-dilated.The device did not pass through a previously deployed stent.Resistance was not encountered when advancing the device.Excessive force was not used during delivery.The stent passed smoothly through the lesion.It was reported that balloon deflation difficulties occurred, and the device would not deflate at the lesion site.The stent was inflated at nominal pressure of 13 atm.When checking the stent with rx it was discovered that the stent was broke.An attempt was then made to remove the stent but the device was difficult to deflate.Pressure had to be applied to retract the stent.There was no major consequences for the patient.The patient was reported to be alive with no injury.
