Event Description
|
It was reported that during an arthroscopy, the implant completely ruptured along the longitudinal axis.Further attempts to adequately fix the halved implant were unsuccessful, so that the complete removal of the implant became necessary, and the patient had to be treated conservatively (the patient was treated without a bone anchor).All the pieces were removed by tweezers.The procedure was completed with non-significant delay.No further complications were reported.

H3, h6: a device deficiency was not identified, and the root cause of the reported event could not be determined since the device was not returned for evaluation.A review of device records showed there were no indications to suggest that the product did not meet manufacturing specifications upon release for distribution.A complaint history review found no similar reported events.The instructions for use were reviewed and found to include conditions of off label use and technique specifics, as well as precautions and warnings related to the use of the device.A risk management review found that the reported failure and/or harm was documented appropriately, and there were no indications to suggest the anticipated risk is not adequate.A review of the material component found that the mechanical and material specifications are documented and a material certificate is required with each lot.Please refer to the instructions for use for recommendations on proper use of the device and potential troubleshooting methods to prevent future reoccurrence of the reported event.No containment or corrective actions are recommended at this time.If the product associated with this event is returned at a future date, this investigation will be reopened for evaluation.

H6, health effect - impact code updated.H3, h6: the reported device was received for evaluation.There was a relationship found between the device and the reported event.A visual inspection of the returned device found that it is not in its original packaging.The fractured implant and insertion device were returned with biological debris on them.The implant is torn into several pieces.Based on the condition of the product material found during visual inspection, additional material testing is not required.A review of device records showed there were no indications to suggest that the product did not meet manufacturing specifications upon release for distribution.A complaint history review found no similar reported events.The instructions for use were reviewed and found to include conditions of off label use and technique specifics, as well as precautions and warnings related to the use of the device.A risk management review found that the reported failure and/or harm was documented appropriately, and there were no indications to suggest the anticipated risk is not adequate.A review of the material component found that the mechanical and material specifications are documented and a material certificate is required with each lot.As of the date of this medical investigation, the requested clinical documentation has not been provided; therefore, there were no clinical factors found which would have definitively contributed to the event.The patient impact beyond the reported events including anchor breakage, device failures or implant rupture (¿along the longitudinal axis¿) is not anticipated as all broken pieces were reportedly removed using tweezers including (¿complete removal of the implant¿).Therefore, no further clinical/medical assessment can be rendered.The complaint was confirmed, and the root cause was associated with unintended use of the device.Factors that could have contributed to the reported event include excessive force on the device, attempted correction of a damaged device, or an inadvertent impact event inconsistent with normal use.Please refer to the instructions for use for recommendations on proper use of the device and potential troubleshooting methods to prevent future reoccurrence of the reported event.No containment or corrective actions are recommended at this time.
