It was reported that this patient with this cardiac resynchronization therapy defibrillator (crt-d) experienced two episodes of ventricular tachycardia (vt) for which this device did not deliver therapy as expected.During the first episode, there was an external shock, but the patient went back into vt but spontaneously converted.During the second episode, the rhythm was detected in a monitor-only zone.The patient was admitted to the hospital.Technical services (ts) noted that this device operated as programmed for both of these events.It was noted that reprogramming was performed.No additional adverse patient effects were reported.Currently, this crt-d remains in service.
